Need IMMEDIATE help w/ email addy for work auth forms

Hi, I am in the process of submitting my work authorization verification forms to West at Home but when I tried to print out the form that contained the options (fax #, email address, address) to submit the notarized form, the printer messed up. I can't access that information now. Does anyone have the email address that I submit the work authorization verification paper work to? Please help ASAP and thanks.
